2.) verification of
malpractice insurance coverage
3.) complete the following
form
4.) submit $95.00 one time application processing fee.

Upon acceptance into the Network, you receive a
provider agreement which establishes your relationship
with Find-a-Therapist, Inc. as an Independent
Contractor.
You will be billed an initial Virtual Office Set Up
Fee of $195.00 and an annual maintenance fee of $125.
There are no other charges required to be part of our

We provide your Virtual Office
which includes Secure Chat capabilities, Online
Appointment Scheduling, Credit Card processing and
secure email accounts. You receive a complete
personal orientation by phone and online to our
policies and procedures.
Support services are available if you need assistance
of any kind.
We manage advertising, promotional campaigns, search
engine submissions and highlighting your online
services once you are accepted into our provider
network.
